Observing, reporting, and documenting resident changes in their health condition will help the entire care team see what may be occurring with a resident since the resident may be unaware of any changes. This course reviews recognizing changes, documenting these changes, and reporting changes to the care team.

The learner will be able to:

• Explain the purpose of reporting and proper documentation of resident changes in condition.
• Recognize the importance of observing resident changes and describing those observations.
• Identify and explain signs and symptoms that need to be reported.
