Purpose:  Aspen Acres Assisted Living, LLC (Aspen Acres) is committed to complying with the provisions contained in Wisconsin Administrative Code DHS 83.  This policy defines and outlines the process that Aspen Acres employees will follow when determining when and how to notify the Department of Health Services of potential abuse.  

Caregiver misconduct and injuries of unknown source are considered “incidents” All staff who have information regarding a possible incident of caregiver misconduct (abuse or neglect of a client or misappropriation of a resident’s property) or an injury of unknown source must report the incident to the Administrator or Designee
